This interdisciplinary team of physicians, nurses, social workers, and chaplains addresses physical symptoms, emotional needs, spiritual concerns, and practical care coordination in hospitals, clinics, homes, and other care settings.
Who Can Benefit from Palliative Care
Palliative Care benefits patients with cancer, heart failure, COPD, dementia, and other chronic conditions, especially those with frequent hospitalizations. Unlike hospice, palliative care can be provide alongside curative treatments from diagnosis forward. Benefits include better symptom control, improved communication, reduced hospital visits, and comprehensive support for patients, families, and caregivers. For patients who no longer choose to receive curative treatment, ALC Hospice provides caring, compassionate care.

Meet Your ALC Palliative Care Team
Nurse Practitioner (NP)
Assesses and manages symptoms, prescribes medications, educates families on disease progression, and coordinates medical care.
RN Case Manager
Primary contact providing monthly assessments, monitoring treatments, and educating caregivers on care techniques.
Physician
Provides medical oversight, manages complex symptoms, certifies eligibility, and supports ethical decision-making.
Social Worker
Offers psychosocial support, facilitates family meetings, coordinates community resources, and provides grief counseling.
Chaplain
Supports spiritual well-being through assessments, counseling, life review work, and rituals regardless of religious affiliation.
Volunteers
Provide companionship, respite care, assistance with errands, and legacy support.
