Who Are We? 

ALC Primary Care@Home is unique from the very first interaction with the patient/caregiver. We are here to honor our patients by providing the services they need and deserve.

– We will respond to new patient referrals the same day they are received.

– We have a social worker on our team who completes a social assessment on all new patients, and acts on anything found during the assessment.

– We have an on-staff RN to provide high quality, disease-specific education and comprehensive triage of acute and chronic conditions.

– Each provider has their own care coordinator to ensure timely follow-up to all issues and needs of our patients and community partners.

– Providers complete their notes within 24 hours of the visit.

We believe that a quality program is holistic and palliative in nature. We believe in the importance of building relationships because with the right information, we can provide the right services at the right time. Building a full picture of a patient – goals of care, social determinants of health – helps us create personalized care plans that focus on the realities of each patient’s situation. 

We partner with fully vetted community resources and vendor partners to ensure they are able to meet the robust needs of all patients under our care.

You place a great deal of trust in us by allowing us to care for you as patient, for a loved one, or for one of the valued residents of your long-term care community. This is not something we take lightly. It is our promise to you that we will strive to meet all medical needs, while understanding that sometimes it is not the medical that drives the need for care.
