Who Are We? 

ALC Primary Care@Home is unique from the very first interaction with the patient/caregiver. We are here to honor our patient’s by providing the services they need and deserve.

– We will respond to new patient referrals the same day they are received.

– We have a social worker on our team that completes a social assessment on all new patients, and acts on anything that is found during the assessment.

– We have an on-staff RN to provide high quality, disease specific education and comprehensive triage of acute and chronic conditions.

– Each provider has their own care coordinator to ensure timely follow-up to all issues and needs of our patients and community partners.

– Providers complete their notes within 24 hours of the visit.

Why do we do this?

We believe that a quality program is holistic and palliative in nature. We can’t provide the right services at the right time if we don’t have all of the information necessary. We believe that knowing the patient goals of care and understanding how the social determinants of health are impacting individual patient allows us to take our care and empathy to the right level.

We partner with fully betted community resources and vendor partners to ensure they are able to meet the robust needs of all patients under our care.

You are placing a great deal of trust in us by allowing us to care for you as patient, a loved one or a facility that allows us the privilege of caring for their residents. This is not something we take lightly. It is our promise to you that we will strive to meet all your medical needs, while understanding that sometimes it is not the medical that drives the need for care, we look at  all aspects of the patient to determine the right path forward.
